Create a New SDM Reunification Assessment

Note: Prior to documenting the SDM Reunification Assessment, it is important that the Household has been documented in N-FOCUS.

  1. Navigate to the Detail Program Case window.
  2. Select the SDM icon.

    Result:  The SDM Household Summary/Detail window displays.

    Note:  This window should have at least one Household Name listed.

  3. Select the appropriate Household Name in the group box.

    Result:  The various push buttons enable, and information in the SDM Household group box displays, including Referral Date and other Household members.

  4. Select the Reunif Asmnt push button.

    Result:  The Detail Runification Assessment window displays.

    Note:  If other Reunification Assessments have been completed for this household, the List SDM Reunification Assessment window displays. You will have to select the New icon to complete a new Reunification Assessment.

  5. Enter the appropriate Assessment Date.
  6. Select the Completed By Out Select arrow.

    Result:  The Search Office window displays.

    Note:  The Office field defaults to the office that corresponds to the current user’s logon ID and position. If you wish to select a different office, continue with Step 7.  If you wish to select the default office proceed to Step 12.

  7. Select the Office Out Select arrow.

    Result:  The Search Office window displays.

  8. Type in the appropriate Office Name.
  9. Select the Search push button.

    Result:  The List Office window displays.

  10. Select the office you wish to view.
  11. Select the blue return arrow.

    Result:  The Search Office Position window displays.

  12. Select the Search push button.

    Result:  The List Office Position window displays.

  13. Select the position that you wish to assign to the Reunification Assessment.
  14. Choose the blue select icon.

    Result:  The Detail Reunification Assessment window displays with the Completed By and Office fields populated.

  15. In the Persons Involved in the Assessment group box, select the child that has been removed.

    Result:  The Removed push button enables.

  16. Select the Removed push button.

    Result:   A ‘Y’ displays in the Removed column for the child selected.

  17. Repeat Steps 15 & 16 for each child removed.
  18. Select the Save icon.

    Result:  The Reunification Assessment ID Number displays.

  19. Select the Risk Index push button.

    Result:  The Reunification Assessment-Risk Index window displays.

  20. Select the appropriate Initial Risk Level or Prevention Level from the drop-down list.

    Note:  The Risk Assessment and Prevention Assessment push buttons are enabled to allow you to review which Risk Level had been previously determined.

  21. Document the answers for the remaining questions on the window.
  22. Select the Narrative push button.

    Result:  The SDM Narrative window displays.

  23. Select the first Narrative Item and enter the Narrative text
  24. Select the Save & Next icon.

    Result:  The next Narrative Type will be selected.

  25. Enter the Narrative text
  26. Select the Save & Close icon.

    Result:  The Reunification Assessment-Risk Index window displays.

  27. Select the OK push button.

    Result:  The Detail Reunification Assessment window displays.

  28. Select the Safety Re-asmnt push button.

    Result:  The SDM Reunif Assessment-Safety Threats/ Decision window displays.

  29. If appropriate, select the appropriate Removal Safety Threats.

    Note:  The Removal Safety Threats are not required.

  30. Select the appropriate Current Safety Threats.

    Note:  If ‘Other (Specify)’ is selected, you must enter text in the Comments text field.

  31. Select the Safety Decision from the drop-down list.
  32. Select the Narrative push button.

    Result:  The SDM Narrative window displays.

  33. Select the first Narrative Item and enter the Narrative text.
  34. Select the Save & Next push button.

    Result:  The next Narrative Type will be selected.

  35. Enter the Narrative text.
  36. Repeat for each Narrative Type listed, then select the Save & Close icon.

    Result:  The SDM Reunif Assessment-Safety Threats/ Decision window displays.

  37. Select the OK push button.

    Note:  If there are no Current Safety Threats, select the No Safety Threats push button.

    Result:  The Detail Reunification Assessment window displays.

  38. Select the PT Summary push button.

    Result:  The Parenting Time Summary List window displays.

  39. Select the first Caregiver Name and double click or select the Open icon.

    Result:  The Detail Parenting Time Evaluation window displays.

  40. Select the appropriate Parenting Time Frequency from the drop-down list.
  41. Select the appropriate Quality of Face-to-Face from the drop-down list.

    Result:  The Parenting Time field displays.

    Note:  If the Parenting Time is ‘Acceptable’, the Overrides group box becomes enabled, if appropriate you can select one of the options.

    Note:  If Discretionary is selected as the Override you must enter the Comments text.

  42. Select the Next push button, if appropriate.

    Result:  The Secondary Caregiver Name displays.

  43. Repeat steps 40 & 41 for the Secondary Caregiver.
  44. Select the Narrative push button.

    Result:  The SDM Narrative window displays.

  45. Select the first Narrative Item and enter the Narrative text
  46. Select the Save & Next push button.

    Result:  The next Narrative Type will be selected.

  47. Enter the Narrative text
  48. Select the Save & Close icon.

    Result:  The Detail SDM Parenting Time Evaluation window displays.

  49. Select the OK push button

    Result:  The List SDM Parenting Time Summary window displays.

  50. Select the Close icon.

    Result:  The Detail Reunification Assessment window displays.

  51. Select the Scoring/Overrides push button.

    Result:  The SDM Reunification Assessment – Score Overrides window displays.

  52. Select each of the Policy Override statements and select either Yes or No
  53. If appropriate, select the Discretionary Override.

    Note:  If Yes is selected, you must also select a response from the drop-down list and document the Override Narrative.

  54. Select the OK push button.

    Result:  The Detail Reunification Assessment window displays.

  55. Select the Recommendations push button.

    Result:  The SDM Reunification Assessment-Recommendations window displays.

  56. Select the Child Name.

    Result:  The Update push button enables.

  57. Select the Update push button.

    Result:  Either the Specify In-care dialogue box displays or the Specify Override or New Goal Dialogue box displays depending upon the Risk Level, Parenting Time and/or Safety Decision.

  58. If you received the Specify In-Care dialogue box, answer 15 of 22 Months question.
  59. Select the OK push button.

    Result:  The Specify Override or New Goal dialogue box displays.

  60. Select the appropriate Discretionary Override from the drop-down list.

    Note:  Certain Discretionary Override selections will enable the New Goal Recommendation drop down.

  61. If appropriate, select the New Goal Recommendation from the drop-down list.
  62. Select the OK push button.

    Result:  The SDM Reunification Assessment-Recommendations window displays.

  63. Select the Narrative push button.

    Result:  The SDM Narrative window displays.

  64. Enter the Narrative text.
  65. Select the Save & Close icon.

    Result:  The SDM Reunification Assessment-Recommendations window displays.

  66. Select the OK push button.

    Result:  The Detail Reunification Assessment window displays.

  67. Select the Narrative icon.

    Result:  The SDM Narrative window displays.

  68. Select Narrative type “Conclusion”.
  69. Enter Narrative Text
  70. Select the Save & Close icon.

    Result:  The Detail Reunification Assessment window displays.

  71. Select Actions – Update Status.

    Result:  The Confirm Save message displays, asking if you want to change the Assessment status to, “Ready for Review”.

    Note:  If required information has not been documented, the Status Change Edit Errors Dialogue box displays. All of the edit errors must be corrected in order to set the status.

  72. Select Yes.

    Result:  The Detail SDM Reunification Assessment window displays with the Status of Ready for Review.

    Note:  Supervisor will be responsible for setting Status to ‘Final’.

  73. Close back to the Detail Program Case window.
