Create a New SDM Safety Assessment

Note: Prior to documenting the SDM Safety Assessment, it is required that the SDM Household has been documented in N-FOCUS.

  1. Navigate to the Detail Program Case window.
  2. Select the SDM icon. (SDM on Yellow Background)

    Result:  The SDM Household Summary/Detail window displays.

    Note:  This window should have at least one Household Name listed.

  3. Select (highlight) the appropriate Household Name in the group box.

    Result:  The various push buttons enable, and information in the SDM Household group box displays, including Referral Date and other Household members.

  4. Select the Sfty Asmnt pushbutton.

    Result:  The Detail SDM Safety Assessment window displays.

    Note:  If other Safety Assessments have been completed for this household, the List SDM Safety Assessment window displays. You will have to select the New icon to complete a new Safety Assessment.  

  5. Enter the appropriate Assessment Date.
  6. Select the Completed By Out Select arrow.

    Result:  The Search Office window displays.

    Note:  The Office field defaults to the office that corresponds to the current user’s logon ID and position. If you wish to select a different office, continue with Step 7.  If you wish to select the default office proceed to Step 12.

  7. Select the Office Out Select arrow.

    Result:  The Search Office window displays.

  8. Type in the appropriate Office Name.
  9. Select the Search push button.

    Result:  The List Office window displays.

  10. Select (highlight) the office you wish to view.
  11. Select the blue return arrow.

    Result:  The Search Office Position window displays.

  12. Select the Search push button.

    Result:  The List Office Position window displays.

  13. Select (highlight) the position that you wish to assign to the Safety Assessment.
  14. Choose the blue return arrow.

    Result:  The Detail SDM Safety Assessment window displays with the Completed By and Office fields populated.

  15. Select the Save icon.

    Result:  The Child Vulnerabilities, Safety Threat, Contact Detail and Status History push buttons are enabled.

  16. Select the Child Vulnerabilities push button.

    Result:  The SDM Safety Assessment-Child Vulnerabilities window displays.

  17. Select all of the appropriate Vulnerabilities in the list.

    Note:  If ‘Other (Specify)’ is selected, you must enter text in the text box and Tab out.

  18. Select the OK push button.

    Result:  The SDM Safety Assessment window displays.

  19. Select the Safety Threat push button.

    Result:  The SDM Safety Assessment-Safety Threats window displays.

  20. Select all of the appropriate Safety Threats in the list.

    Note:  If the first Safety Threat is selected, a subsequent Threat listed under it must also be selected.

    Note:  If the Safety Threat, “Child shows signs of emotional harm…” is selected, a subsequent Threat listed under it must also be selected.

    Note:  If ‘Other (Specify)’ is selected, you must enter text in the text box and Tab out.

  21. Select the OK push button.

    Note:  If there are no Safety Threats, select the ‘No Safety Threats’ push button.

    Result:  The Safety Assessment window displays.

  22. Select the Contact Detail push button.

    Result:  The Contact Detail window displays.

    Note:  Contact Detail is required to be completed if an Intake has been tied to the Safety Assessment.

  23. Select a name from the Person drop down list.

    Note:  If appropriate, click on the “Not Contacted” check box and select the appropriate Reason from the drop down list.

    Note:  Contact Detail must be documented for at least one victim of the tied Intake.

  24. Select the Contacted By from the drop down list.
  25. Enter the Date of the contact.
  26. Enter the Time of the contact.
  27. Enter the information regarding Location of the contact.
  28. Enter the information regarding Others Present.
  29. Enter any other Comments regarding the contact.
  30. Select the Add push button.

    Result:  The Contact Information fields are cleared and the information displays in the Contact Detail box at the bottom of the screen.

    Note:  You may now add other Contact information for the same person.

    Note:  For error correction, highlight the appropriate line in the Contact Detail box, make the changes in the narrative boxes and select the Update pushbutton.  You may also Delete the narrative.

  31. Select the Save & Next icon to record Contact Information for the other persons in the Person list.
  32. Select the Save & Close icon.

    Result:  The Safety Assessment window displays.

  33. Select the Safety Intervention push button.

    Note:  If ‘No Safety Threats’ was previously selected, the Safety Intervention  push button will not be enabled.

    Result:  The SDM Safety Assessment-Safety Interventions window displays.

  34. Select all of the appropriate Safety Interventions in the list.

    Note:  If ‘Other (Specify)’ and/or ‘Other Court Action (Specify)’ is selected, you  must enter text in the appropriate text box and Tab out.

  35. Select the OK push button.

    Result:  The Safety Assessment window displays.

  36. Select the Safety Determination push button.

    Result:  The Safety Assessment – Safety Decision window displays with the  list of children in the SDM Household and their Safety Determination identified.

    Note:  If in the Safety Intervention, you selected the intervention , “child must be removed from the home…”, all of the children in the list will be designated as ‘Unsafe’. You must highlight which children will remain in the home and select the Safety Determination push button and select ‘Conditionally Safe’ from the drop-down list.

  37. Select the OK push button.

    Result:  The Detail SDM Safety Assessment window displays.

  38. Select the Intake out select arrow if the Safety Assessment should be tied to an Intake.

    Result: Search Intake window opens. Search for the appropriate intake and use the blue return arrow to complete the tie.

    Note: Supervisors have the security to tie intakes to an SDM Safety Assessment when the Safety Assessment is in final status.

  39. Select the Save icon.
  40. Select Actions – Update Status.

    Result:  The Confirm Save message displays, asking if you want to change the Assessment status to, “Ready for Review”.

    Note:  If required information has not been documented, the Status Change Edit Errors Dialogue box displays. All of the edit errors must be corrected in order to set the status.

  41. Select Yes.

    Result:  The Detail SDM Safety Assessment window displays with the Status  of Ready for Review.

    Note:  Supervisor will be responsible for setting Status to ‘Final’.

  42. Close back to the Detail Program Case window.
