UNCOPE Assessment

The UNCOPE Assessment tool is created to assess substance abuse issues for adults who are part of a household that is part of a CFS initial assessment.  

1. Navigate to the SDM Household Summary/Detail window. Highlight the SDM Household, which enables the UNCOPE icon.
2. Click the UNCOPE icon.  If there are prior assessments for adults in the SDM Household,  the List UNCOPE Assessment window opens.  Click the New icon.  The Detail UNCOPE Assessment window opens.
3. Use the Out Arrow to select the name of the person in the SDM Household that the UNCOPE assessment will be completed for. The Detail UNCOPE Assessments Household Persons dialog box opens.  Highlight the person.  Click OK to return.
4. Use the Out Arrow to enter the Completed By information. The Search Staff Person window opens. Locate the Staff Person, click OK and you will be returned to the previous window.
5. Enter the date the UNCOPE assessment is completed.
6. Select either Yes or No from the drop down list next to each question.  Enter comments in the Comments box, if needed.
7. Select Save and Close to return to the List UNCOPE Assessment window.
