Disabilities Waiver

Disabilities Adult Comp Waiver

Disabilities Adult Day Waiver

Shared Data:
- Although every DD case is a single person program
case, other persons and other program cases may be a part of a shared
Master Case.
- It is important for those responsible for DD case
maintenance to identify all Master Cases in which their DD client is a
member. Notify any PSW or SSW who is assigned to an active program case
when a waiver program case has been added.
- It is also important for the PSW and the SSW to
note any DD client who has ever been a member in one of their Master Cases.
Then inform one of the DD contact persons (Pam Hovis
@ 479-5247, Kate Mathias
@ 479-5592, or Lelia
Razey @ 479-5295)
- Changes in client address, phone numbers, living
arrangements, etc., are the common responsibility of both the DD staff
member and the PSW or SSW staff member.. Communicate via telephone or
e-mail to inform the other worker of the change and to arrive at a plan
for updating the common data.
- Closing
a Medicaid Case or changing it from “Active” to “Spenddown” when a CDD
or ADD Waiver client is tied to it causes the Waiver case to close. This
will cause the service authorizations attached to the Waiver case to close
as well. This occurs
whether the Medicaid is an Expert System MED eligibility determination
or whether the Medicaid case eligibility is determined by the worker and
entered as a Mainframe Medicaid case. Mainframe MED cases include the
CFS program cases with Medicaid categorical codes as well as the other
CWIS related SA/Med, SG/Med, Med-SA
and Med-SG
cases. They also include the Kids’ Connection (Med-KC)
cases.
The
PSW or the SSW staff person must be sure to inform the DD staff person
that the Medicaid case will be closing. This will allow the DD staff member
to take whatever steps are necessary to continue the services if appropriate.
Communication between DD staff and the Worker responsible for
the Medicaid (PSW or SSW) is crucial.
