Medicaid Card Issuance

Medicaid card issuance will occur once; it will be a laminated card, similar to private insurance cards.

Information on the card will include:

• Name(s).  The card will display a maximum of six names; a second card will be issued if more than six clients are in the case.
• Medicaid claim number
• Issue date
• DHHS logo
  

Information on the back of the card directs providers and clients to toll-free numbers and web sites for inquiry on current eligibility

New cards are issued in the following circumstances

1. Opened as a new participant.
2. A role change occurred from financially responsible or community spouse to a participant.
3. Living arrangement change from a nursing home to any other living arrangement.
4. Change from QMB to Medicaid coverage.
5. Unborn is updated to born.
