Tribal Membership Inquiry Letter

Tribal Membership Inquiry Letters are available for determining tribal membership. Use the letter when a child or parent identifies themselves as belonging to a Tribe. Used by CFS Staff, tribal information must be created first.  That is a security driven function.

Note:  Before creating the Tribal Inquiry Letter/Form be sure you have documented the appropriate information on the Detail Tribal Information window.

Note:  Before creating the Tribal Inquiry Letter/Form have all of the necessary information available, as the window does not save information previously entered.

  1. Navigate to the Detail Program Case window.
  2. Select the Correspondence icon.

    Result:  The Search for Correspondence window displays.

  3. Select the New push button to create new correspondence.

    Result:  The Create Correspondence window displays.

  4. Select the Tribal Membership Inquiry Form.
  5. Select the OK push button.

    Result:  The Tribal Membership Inquiry Form window displays.

  6. Select (highlight) the name of the appropriate child.

    Result:  Information about the child that has been previously entered on N-FOCUS  will populate various fields.

  7. Select the appropriate Court from the drop down list.
  8. Enter Place of Birth information if not defaulted, and if known.
  9. Select  the name of the Tribe that this document will be sent to.

    Note:  You may multi select if the document is to be sent to more than one Tribe.

    Note:  If there is additional information regarding the child’s affiliation to a particular tribe, double click on the tribal name and a text box will display, allowing you to enter the information. This information will print in the Inquiry Letter.

    Note:  If a Tribe you wish to send the Tribal Inquiry Letter/Form to, does not appear on the list, select the Add pushbutton. You can then select additional Tribes or type in the name of an additional Tribe.

  10. Select (Highlight) Father in the Ancestry Information box.

    Result:  the Details pushbutton enables.

  11. Select the Details pushbutton.

    Result: Ancestry Detail Information dialogue box displays.

  12. Enter all information that is known regarding the child’s father.

    Note:  If you do not have any information regarding the father or any of the other relatives, leave all the fields blank.  Do not enter Unknown.

  13. Select the Next pushbutton.

    Result: The Ancestry Detail Information dialogue box regarding the Mother displays.

  14. Enter all information that is known regarding the child’s mother.

    Note:  If the mother’s Current and Previous Addresses are the same as what was entered for the father, use the Out Select Arrow to select the address.

  15. Using the Next pushbutton, you can continue to enter the Ancestry Information for the other family members.

    Note:  In order to help determine if the child is a member of the Tribe, it is important to document as much Ancestry Information as possible for all family members.

  16. When you have completed the known Ancestry Information for the family members, select the Finish pushbutton.

    Result:  The Tribal Membership Inquiry Form window displays.

  17. Select the Indian Custodian pushbutton.

    Result:  The Indian Custodian dialogue box displays.

    Note:  If the child has an Indian Custodian, it is important to document this Information, so that the Inquiry Form is sent to them as well.

  18. In the Indian Custodian dialogue box enter the person’s name and address information.
  19. Select the Add/Update to the List pushbutton.

    Result: The person’s name will display in the upper box.

  20. Select (highlight) the name in the upper box and select the OK pushbutton.

     Result:  The Tribal Membership Inquiry Form window displays.

  21. Select the Checklist pushbutton.

    Result:  the Checklist dialogue box displays.

  22. Check which documents will be attached to the Tribal Inquiry Letter/Form.

    Note:  A Copy of the Petition that made the child a State Ward must always be attached, so here just enter the date of the petition. If any of the other boxes are checked, the date must also be documented.

  23. Select Save.

    Result: The Print dialogue box displays.

    Note:  When there is more than one child, the Tribal Inquiry Letter/Form must be sent separately for each child. If the all the family information is the same for the other children, it does not have to be reentered.

    Highlight the second child and select Save to print the form. Repeat for other children. When the last child has been printed, select Save & Close.
