This is used to request verification when the information is not available on Vital Statistics Records. This can also be used to request certified copies of Vital Statistics records so this information can be used in court.
Key Points:
Select a person on NFOCUS, and navigate to the Interface Menu. Select the VS icon. Then select the Request for Verification and Certificate tab. Select the type of certificate (Birth, Death, Marriage or Divorce) that you are requesting. Some fields will be displayed from the NFOCUS Person Detail window. You can update any of this information as needed.
If you need to prove relative responsibility, select the child for whom you need to prove relationship. Change the child’s name and information to the relative’s information to request a birth verification on the relative.
When completed, select one of the following pushbuttons:
Verification: Select if the person wasn’t on the Vital Statistics Records and citizenship needs to be verified. When this information has been verified, you will receive an alert. This process is paperless. Do not print this document. You will be able to continue to view it online.
Certified In-State: If a Nebraska certified certificate is needed for court or some other authorized purpose. Do not print this document. Central Office staff will handle all requests with Nebraska Vital Statistics. The paper document will be returned to you when it is received from Vital Statistics.
Out of State: Select if certificate being requested is from another State. You will need to print this document so an ASD-46 can be attached. An ASD-46 must be attached to all requests going to another State. If this is not attached, the request will be returned to you. Once the ASD-46 has been attached, route this document to Central Office. Some States require additional information be provided. A document may be returned to you to attach some additional information, if required from the State where the certificate is located.
Once created and submitted, the request goes into a Pending status until acted on by Central Office staff. Central Office staff will update the status. The statuses are listed below. To view the current status of the request:
