Recoupment of the Error

Once the worker identifies that there was an error in Medicaid benefits and determines the amount of the error, s/he must complete Form IM-64.

The worker should make only one request. Unless the error involves a fraud referral and investigation, the client is under no obligation to repay the error.

If the client repays the error, the worker documents in the case record the amount of repayment and forwards the money to the Central Office, Finance and Accounting, via Form FA-65.
