The following fields at the bottom of the SS-5 must be completed by the worker completing the form:
NPN: This field must be completed accurately in order to ensure that the return record on the tape from SSA can be processed to the correct client. This information is entered by SSA into the system and returned on the tape and used by HHS to locate the right client to update the SSN and/or Validation Code.
The first two digits are always 28 which is the state code for Nebraska. This identifies the state where SSA should send a return record on the Enumeration tape. The next nine digits should be the client's own individual SSN or if s/he has not furnished one, the random number assigned by the WCE or FSP system. Do NOT send a case number/ID or any other identifier. There should be a space (but not a dash) between the state code and the SSN/random number. A number 0 should have a slash through it to distinguish it from the letter O. Example: 28234567890
If you are sending an SS-5 on a newborn, be sure to change the child's information on WCE before you submit the SS-5--Unborn to name of the child and the Expected Date of Delivery to the actual birthdate. When we process the Enumeration tape, we not only use the number in this NPN field but we also use the name, sex, and DOB to make sure we update the correct client.
PBC: This is the Primary Birth Certificate field. This must also be coded "N".
EVI: This is the Evidence of Identity field. This should be coded either "Y" for yes or "W" when the individual is a child age 6 or younger and the birth certificate is being used for both age and identity.
EVA: This is the Evidence of Age field. This should be coded either "Y" for yes or "N" for no.
Original SSN: Age must be verified for an original SSN.
Replacement SSN: If changes are being made it need only be verified if you are changing the DOB.
EVC: This is the Evidence of Citizenship field. This should be coded "Y" for yes or "NO for no. SS-5's should be completed only for U.S. citizens.
Original SSN: This must be verified for an original SSN.
Replacement SSN: If you are changing data on the SS-5, e.g., name or DOB, this could be coded "N".
