If the client has not applied within 30 days of the date s/he is given the Referral for Social Security Number Application, the worker must not include the client in determining the size of the assistance unit. Before taking adverse action, the worker must take into consideration the client's ability to follow through on the referral (such as lack of transportation, no visit by SSA to the contact station, lack of rrequired verification documents, etc.) and use prudent person principle.
To refer the client to the Social Security office, complete Form IM-68, "Referral for Social Security Number Application." Give the client the original and file the copy in the case record. The client is required to apply for an SSN within 30 days of the date s/he is given Form IM-68. The Social Security office will complete the bottom portion of Form IM-68 and return it to the local office. In addition, SSA will issue Form SSA-5028 as verification of application if the client requests.
