The state medical review team evaluates the appropriateness of each SNF, ICF, or ICF/MR client's medical and social care needs, and may, after collecting pertinent data, recommend de-institutionalization using Form DPW-100, "De-Institutionalization Referral." The recommendation must include information about the client's physical/mental condition and needs. See 471 NAC 12-008.12.
