Upon receiving Form DPW-100, the worker shall visit the client and/or representative, discuss the de-institutionalization recommendation, and present written notification of the recommendation. (The worker may contact the facility''s discharge planner for assistance in notifying the client before this visit.) To discuss the client's needs and preferences, the worker shall arrange a meeting with the client and/or representative; his/her income maintenance or social services worker, if possible; the facility's discharge planning staff; and any interested family members.
