5-015.11D Investigation Procedures
473 NAC 5-015.11D
To conduct the investigation, the worker shall -
1. Visit the residence of the allegedly abused/neglected adult, whenever possible and feasible. The worker shall determine whether any immediate action is needed to protect the adult (see 473 NAC 5-015.11C);
2. Interview the allegedly abused/neglected adult in order to obtain his/her explanation of the incident(s) alleged in the complaint;
3. Complete Form DSS-76, "Client Assessment Tool," to determine the adult's functioning in his/her environment as demonstrated by his/her -
a. Understanding of any dangers and need for protective/corrective action;
b. Ability to provide for adequate shelter, clothing, and food;
c. Need for medical treatment and ability to procure and/or consent to such treatment;
d. Financial situation (e.g., sufficient income or money management ability);
e. Ability to manage household tasks;
f. Ability to interact with others (e.g., whether s/he endangers or threatens others' property/physical well-being or exhibits inappropriate behaviors);
g. Ability to control own behavior so as to not harm himself/herself;
h. Need for legal services such as guardianship and mental health commitment;
i. Willingness to accept non-legal assistance such as voluntary mental health treatment or social services; or
j. Ability to maintain the home in an adequate condition, free from environmental hazards;
4. As appropriate, administer the Mini Mental Status test or the Goldfarb;
5. Interview other individuals involved in the situation, if any, in order to obtain their explanations of the incident(s) alleged in the report;
6. Contact and interview interested individuals (e.g., guardians, caregivers, and relatives), when appropriate;
7. Make collateral contacts as appropriate (e.g., the reporter, landlord, neighbor, physician, hospital staff, law enforcement), to determine facts about the nature and cause of the alleged abuse/neglect;
8. Obtain necessary medical information;
9. Request State Fire Marshal and Department of Health inspections of the adult's living arrangement, as appropriate, when the adult's living arrangement presents a hazard; and
10. Determine whether the report is unfounded or whether continued involvement is necessary.
