The worker shall complete Form DSS-60, "Adult Abuse/Neglect Report," and submit it in an envelope labeled "Confidential" to the Central Registry for Abused/Neglected Adults for each investigation conducted. Form DSS-60 must be submitted within 15 calendar days following the close of the investigation.
