5-015.15 Arranging/Coordinating Health and Social Services

473 NAC 5-015.15

When arranging/coordinating health and social services, the worker shall involve the client or his/her legal representative whenever possible. This component includes -

1. Locating or assisting the client to locate needed service providers;

2. Providing information to the service provider(s) concerning the client's need for the service;

3. Providing guidance to the client concerning the reason for the referral to the service resource; and

4. Providing follow-up contact with the service provider and the client. The worker shall -

a. Determine whether the service is meeting the client's needs; and

b. Determine if the client is experiencing problems using the service and explore with the client solutions to problems identified.

 

The worker shall use supportive community resources when available.
