4-004.01C4 Change Report Forms

The household will receive a Change Report Form every time it receives an automated food stamp notice.

 

The worker acts on changes reported over the telephone, by mail, or in person by the household in the same manner as those reported on the change report form.

 

Examples of changes that must be acted upon for households assigned to Change Reporting even though the client is not required to report the change within the certification period:

1. High school student with earned income turns age 18;

2. Individual is disqualified due to intentional program violation;

3. Person becoming eligible for medical and unlimited shelter deduction when turning age 60; and

4. Households are not required to report changes in assistance grants as the worker is already aware of the change.

 

Workers must follow these procedures to document and act on reported changes:

1. Document the date the change was reported, who reported it, how the information was provided (by phone, fax, in person, etc.) and the details of the change reported (new job, move, addition or loss of household member, etc.) even if there is no change in the allotment;

2. Notify the client in writing of the verification needed for the reported change, the time limit for providing and the consequences of not providing the verification;

3. Document the action and the date action was taken as a result of the reported change; and

4. Notify the household if the change:

a. Does not affect the household's eligibility and level of benefits; or

b. Affects the household's eligibility and level of benefits.
