Disaster SNAP Application

Persons living in Nebraska counties and/or Zip Code(s) that have been declared eligible for disaster benefits are potentially eligible for Disaster SNAP benefits.

Information to assist you in completing the application:

  •  Head of the Household is the usually the person completing the application, however if you are applying on behalf of someone, it would be that person.
  •  The Home Address is where you were living at the time of the disaster.
  •  The Mailing Address is optional, but could be where you are currently living or the address your mail is going to at this time.
  •  An Authorized Representative is a person that you are giving permission to apply for benefits for you and/or use the benefits on your behalf.

If changes need to be made to your answers, there is a summary page where “CHANGE” can be selected in each section of the application summary.  You will be taken to that section to make the change, and returned to the summary page after the change is completed.

EXIT:  Select the Exit button to close the application without saving any information.

CONTINUE:  Select the Continue button to continue processing the application.

SUBMIT SECTION:  When you submit the application, an electronic signature is created which is as valid as a handwritten signature.  Your signature should be your first and last name, as you normally sign it for other business concerns.  

The confirmation number is unique to your application.  Please keep the number to refer to when communicating with the Department of Health and Human Services office.  It is called the Application ID number when received in the local office

 An interview is needed to determine eligibility.  Go to a Disaster Center to complete an interview.   At your interview you will need to provide identification and proof of your residence at the time of the disaster.
