The Expense Section asks you to provide information on all expenses incurred by the members of your household.
You may be asked to provide verifications of the expenses. Some expenses that are not verified will not be counted as deductions in the budget.
In this section, you will be asked to give us information about your household’s expenses.
If anyone pays rent or mortgage, we will ask for the amount and ask if anyone helps pay this bill. If you rent your home, we will also need the name and phone number of your landlord. We will also ask about utility bills.
If anyone is paying child support, we will ask about the court order for the child support including the amount you pay and the court order number.
If anyone is billed for child care, we will ask about the amount and the provider’s name and address.
Failure to report or verify an expense will be seen as a statement by your household that you do not want to receive a deduction for the unreported and/or unverified expense.
Housing Expense: Select Add Housing Expense to open the next window. Select the person(s) who are responsible for the housing expense; then select from the drop down list the type of expense. Enter the amount of the expense, even if you are not able to currently pay this expense. If Rent was selected as an expense, enter the name of the landlord and his/her phone number.
Is this public/subsidized (Section 8) housing: Select Yes or No.
Does the person billed for this expense have an eviction notice: Select Yes or No.
Utility Expense: If the household is responsible for paying utilities, select Add Utility Expense. On the next window, select the person(s) responsible for the utility expenses. Select the type of utility expense from the drop down list. If the utility for either or both heating and cooling, check the boxes to the left of the item. Select the dwelling type from the drop down list.
Supplier Address: The supplier's name and address is the name of the company providing the utility service. Also enter the name on the account and the account number. If someone outside of your household helps pay the expense, enter their name.
If the utility has been shut off, or there is a shut off notice: Select Yes or No.
Are you out of heating fuel: Select Yes or No. Click ADD ANOTHER to enter information about addition utility expenses.
Child Support/Alimony: This is a legally obligated payment made on behalf of a household member's children who are not living in your home. The child support payment must be court ordered. Select Add Child Support/Alimony. On the next window, select the person who is obligated to pay the expense. . Enter the Court Order number. Select the state and county the order was issued in by selecting the state from the drop-down arrow, and enter the name of the county in the next box. Enter the amount of support ordered in the Amount Ordered box, and enter the amount being paid in the Amount Being Paid box. If there is more than one support order, select ADD ANOTHER to enter the information for the additional order or arrearages..
Child/Dependent Care: This is care for care for minor children or other dependents included in this application. Select Add Child/Dependent Care. Select the person paying the care, enter the amount paid and how often (frequency). If a person not in your household helps pay the expense, enter their name in the box. If more than one expense is paid, click the ADD ANOTHER button to open a new window.
Medical Expenses: If anyone pays for insurance that pays for expenses or loss incurred for medical reasons, as through illness or hospitalization. This includes policies for prescriptions, dental coverage, cancer, nursing home care, etc. These policies could be through work, the military or paid for by another person.
Select Add Medical Expense. On the next window, select the person who is responsible for payment of the expense, then select the type of medical expense from the drop down list. If someone outside of the household pays or helps pay for the medical expense, enter the person's name in the box.
If additional medical expenses need to be added, click the ADD ANOTHER button to enter information about the next type of expense.
Utility Bill Pay Assistance: If you or anyone in the household received help paying heating and/or cooling expenses in the past 12 months, select Add Utility Bill Pay Assistance. Select the person who received the assistance; enter the address where the person was living (if different from the address on the application) when the help was received, and select who provided the help. Click ADD ANOTHER to enter information about addition utility expenses.
