Birth Request Detail

WINDOW OVERVIEW:

The Birth Request Detail window displays the information entered in the search for the birth certificate. If necessary, the request can be updated on this window.

FIELD DESCRIPTIONS:

Request Type:This box displays the type of certificate requested.

Person Number:This box displays the agency related person number assigned to the person.

Certificate Number:This box displays the assigned certificate number to the requested certificate.

Status:This box displays the current status of the request. This field can be updated by selecting another status from the drop down list.

Status Date:This box displays the effective date for the status. To update enter the date of select the calendar icon to open a calendar of the current month.

Note: The boxes below display when it is an out of state request.

Warrant Number:This field displays the number on the check sent to the other state.

Warrant Date:This field displays the date the check was created.

Warrant Amount:This field displays the amount of the check.

Child Box:

First Name:This field displays the child's first name as listed on the previous request.  This is a required field.

Middle Name:This field displays child's middle name if they have one.

Last Name:This field displays the child's last name. This is a required field.

Ext:This field displays the child's name extension
Jr. Sr. III. etc. if they have one.

Birth Date:This field displays the child's date of birth (MM-DD-YYYY). This is a required field.

City:This field displays the name of the city where the child was born. This is a required field.

State:This field displays the name of the state where the child was born.

County:This field displays the county where the child was born.

Foreign Country:This field displays the foreign country where the child was born if request is out of state and applicable.

Father's Name Box:

First Name:This field displays the child's fathers first name. Required for out of state requests.

Middle Name:This field displays the child's father's middle name if he has one.

Last Name:This field displays the child's fathers last name. Required for out of state requests unless the mother's name is entered.

Ext:This field displays the child's father's name extension
Jr. Sr. III. etc. if he has one.

Mothers Name Box:

First Name:This field displays the child's mother's first name. Required for out of state requests unless the mother's name is entered.

Middle Name:This field displays the child's mother's middle name if she has one.

Last Name:This field displays the child's mother's last name. Required for out of state requests unless the mother's name is entered.

Ext:This field displays the child's mother's name extension
Jr. Sr. III. etc. if she has one.

Maiden Name:This field displays the child's mother's maiden name.

Request History Box:

Status Date:This column displays each date a change was made to the request.

Status:This column displays the status of the request.

Status Reason:This column displays the reason for the status if it was rejected.

Create User Id:This column displays the name of the staff person that created or updated the request.

ACTION BUTTONS:

	Select the Print button to open the Print window so the page can be printed.
	Select the Cancel button to cancel any changes made to the window information.
	Select the Update button to add any changes made to the window information.
