Divorce Request Detail
WINDOW OVERVIEW:
The Divorce Request Detail window displays the information entered in the search for the divorce certificate. If necessary, the request can be updated on this window.

FIELD DESCRIPTIONS:
Request Type:This box displays the type of certificate requested.
Person Number:This box displays the agency related person number assigned to the person.
Certificate Number:This field displays the certificate number after it is received.
Status:This box displays the current status of the request. This field can be updated by selecting another status from the drop down list.

Date:This
box displays the effective date for the status. To
update enter the date of select the calendar icon
to open
a calendar of the current month.
Husband Box:
First Name:This field displays the groom's first name. This is a required field.
Middle Name:This field displays groom's middle name if he has one.
Last Name:This field displays the groom's last name. This is a required field.
Ext:This field displays the groom's name extension if he has one.
Wife Box:
First Name:This field displays the bride's first name. This is a required field.
Middle Name:This field displays the bride's middle name if she has one.
Last Name:This field displays the bride's last name. This is a required field.
Ext:This field displays the bride's name extension if she has one.
Maiden Name:This field displays the bride's maiden name.

Date:This
field displays the date of the marriage. This
is a required field. The calendar icon
may be used to find
the date.
City:This field displays the name of the city where the couple married. This is a required field.
State:This field displays the name of the state where the couple married.
County:This field displays the county where the couple married.
Request History Box:
Status Date:This column displays the each date a change was made to the request.
Status:This column displays the status of the request.
Status Reason:This column displays the reason for the status.
Create User Id:This column displays the name of the staff person that created or updated the request.
ACTION BUTTONS:

Select the Print button to open the Print window so the page can be printed. |

Select the Cancel button to cancel any changes made to the window information. |

Select the Update button to add any changes made to the window information. |
