Search Requests

WINDOW OVERVIEW:

The Search Requests window is used to search certificate requests based on the parameters entered.

It is opened when the Interface icon is selected on the Main Menu and the Vital Records icon is selected on the Interface Menu.

Note:If a case person is selected prior to opening Vital Records, you will only see requests for that specific person and no Search Requests window is available.

Request Details

FIELD DESCRIPTIONS:

Request Type:Enter the request type to search on (All, Certified Copy, Out of State Copy, or Verification). This is a required field.

Status:Enter the status to search on (All, Certificate Sent to Worker, Mailed to Vital Stats, Pending, Received from Vital Stats, Rejected, or Verified). This is a required field.

Certificate Type:Enter the certificate type to search on (All, Birth, Death, Divorce, or Marriage). This is a required field.

Requester/User ID:Enter the requester/user id.

Last Name:Enter the last name of the person.

First Name:Enter the first name of the person.

Event Date Range From:Enter the event date range from.

Event Date Range To:Enter the event date range to.

ACTION BUTTONS:

 Select the Go button to begin the record search process.
 Select the Reset button to remove the search criteria.
