The BDE Demographics window displays identifying information about the person applying for or receiving Social Security benefits.

SSN: This box displays the N-FOCUS Social Security Number for the person.
Birth Date: This box displays the person's N-FOCUS date of birth.
Name: This box displays the person's N-FOCUS name.
Gender: This box displays the person's N-FOCUS gender code.
Bendex Name: This box displays the name used by the Social Security office to identify the person.
Date of Birth: This box displays the birth date the Social Security office is using to identify the person.
Date of Death: This box displays the person's date of death used by the Social Security office.
Input SSN: This box displays the Social Security Number used by the state to request the BDE information for the person.
Gender: This box displays the gender code used by the Social Security office.
Date of Birth Proof: This box indicates if Social Security has verified the persons birth date.
Date of Death Proof: This box indicates if Social Security has verified the persons date of death.
Beneficiaries Own SSN: This box displays the person's Social Security number. This will be different from the Claim Account Number if the person is receiving benefits through a spouse or parent's record.
Payee Name: This box displays the name of the person listed as the payee for the benefits. This person can be the beneficiary or someone acting on their behalf.
Address: These boxes display the address of the payee. The address includes street, city, state and zip code information.
Citizenship Information:
This area can display up to three historical occurrences of citizenship.
Start Date: This box displays the date the person's citizenship started in the country listed in the box below.
Country: This box displays the country where the person has citizenship.
Stop Date: This box displays the date the person's citizenship in the listed country ended.
US Citizenship Proof: This box indicated if the citizenship in the US is verified.
