WINDOW OVERVIEW:

This window allows the entry of data regarding a Case Person's attested citizenship, documented non-citizenship or undocumented citizenship information regarding Citizenship/Immigration information for Case Persons within a Medicaid Program Case. This window is accessed from the Detail Master Case window, Actions>US Citizenship and Immigration

FIELD DESCRIPTIONS:

Master Case ID:  This field displays the unique, system-assigned number used to identify each Master Case.

Name: This field displays the name of the person whose name the Master Case is registered.

Medicaid Persons Group Box:

Last Name:  This column displays the last name of each member of the Pending or Active Medicaid case.

First Name: This column displays the first name of each member of the Pending or Active Medicaid case.

M: This column displays the middle initial of each member of the Pending or Active Medicaid case.

Ext: This column displays the name extension of each member of the Medicaid Case. Example: JR, III, IV, SR

Click this button to access the Person Detail for the selected Medicaid Person.
Click this button to access the Interface window for the selected Medicaid Person.

RADIO BUTTONS:

Attested US Citizen: Select this radio button when the Case Person selected from the Medicaid Persons Group Box has attested to their US Citizenship on the Application for Assistance form.

Documented Non-Citizen: Select this radio button when the Case Person selected from the Medicaid Persons Group Box has documentation indicating they are a Non-Citizen.

Undocumented Non-Citizen: Select this radio button when the Case Person selected from the Medicaid Persons Group Box is an undocumented non-citizen.

Determine As Of Date: Enter the date you wish Citizen/Non-Citizen status determined as of. This field will automatically default to the Current Date when this window is in Add mode. The date can be changed to an earlier date.

Document Details Group Box:

Document Type: Select the appropriate document type from the drop down list.

Note: The options provided in the drop down list will be based on the radio button selected above (Attested US Citizen, Documented Non-Citizen, Undocumented Non-Citizen). Once a document type is selected, the fields show below will change based on the selection made.

Document Expire Date: The date the selected Document Type expires.

Passport Number: The Case Person's Passport number.

Visa Number: The Case Person's Visa number.

I-94 Number: The Case Person's I-94 number.

Alien Number: The Case Person's Alien Number.

Card Number: The Case Person's Permanent Resident Card Number or Employment Authorization Card Number.

Passport Country of Issuance: The country that issued the Case Person's passport.

SEVIS ID: The Case Person's SEVIS ID number.

Naturalization Number: The Case Person's Naturalization Number.

Certificate of Citizenship Number: The number shown on the Case Person's Certificate of Citizenship.

Other Document Description: This is a free form field available to enter additional information.

ACTION BUTTONS:

Click this button to clear the fields within the Document Details Group Box.
Click this button to view the history of DHS/SAVE requests made.
Click this button to submit the information to the DHS/SAVE Interface for verification of alien status. This button is enabled when Documented Non-Citizen is selected.
Click this button to submit the information to the SSA Interface for verification of citizenship. This button is enabled when the Attested Citizen is selected.

 

Response Status Messages and Suggested Actions:

Error in Processing Request: Close the request, make sure document information is correct and select the Submit to DHS/SAVE Interface again.
Awaiting G-845 Response: A pre-filled PDF of for G-845 should be on the Interface window. The worker should print the form and contact the client to request what documents the G-845 is requesting. When the documents are received attach them to the G-845 and mail to the address specified on the form. Enter the G-845 sent date on the Interface window to alert DHS that the information has been mailed.
Correction Needed-Document ID Incorrect: Update document information and select the Submit to DHS/SAVE button.
