Early Development Network Referral

WINDOW OVERVIEW:

The Early Development Network Referral correspondence is used to create a referral to an Educational Service Unit as required by the Federal Child Abuse Prevention and Treatment Act (CAPTA 1988).  

Creating a referral electronically ensures that the most recent information is included and a record of the referral is maintained for future use.

The referral is opened using the Correspondence icon or Forms option in the Goto drop down menu on the Detail Program Case window.

FIELD DESCRIPTIONS:

Referred Organization: This is a required field. The identified Educational Service Units are included in a list. To select the appropriate Organization click on the down arrow in the selection box and highlight the Organization closest to the address where the child being referred lives.

Child Group Box:

Name:  Select the arrow to select the appropriate Child from the Early Development Networ - Select Child pop up window.

Note: This list displays all children in the case.  Highlight the name of the child who is being referred.  Only one child can be selected at a time. The child's Birth Date, Sex, Address, Legal Status, Placement Type, Facility Type and Placed with fields will be populated. If a child does not appear on the list make sure that their Birth Date is entered correctly.

Birth Date:  This field populates automatically after child is selected.  Information can be updated on the Person Detail window.

Sex: This field populates automatically after child is selected.  Information can be updated on the Person Detail window.

Child’s Address Group Box:

The Child's current address automatically populates based on current address.  The fields can be updated on this window but these changes do not update the system information.

Line 1: This field displays the Child's street address for a physical address or the P.O. Box for a mailing address.

Line 2: This field displays any additional street address information such as an apartment number for a physical address. This is an optional field.

Line 3: This field displays any additional street address information such as an apartment number for a physical address. This is an optional field.

City: This field displays the city where the address is located.

State: This field displays the state where the address is located.

Zip: This field displays the zip code for the address.

Phone #: This field displays the child's current phone number.

Legal Status: The current Legal Status for the child.

Placement Type: The current Placement Type for the child.

Facility Type: The facility type for the child's current placement.

Placed With: The individual or organization with whom the child is placed.

ACTION BUTTONS:

Select this button to enter information about the Child's Non-Custodial Parent. Enter address, Home and Work phone numbers.
Select this button to access the Early Development Network Risk Factors window. Select the Child's Risk Factors, potential risk factors that may impact Early Development Network personnel and document an other potential risk factors not listed.
Select this button to enter the child's doctor address, phone number and last doctor's appointment date. Text area is also available to document any additional developmental concerns, any medical concerns and any additional comments.
Select this button to document the parental rights and educational surrogate information.

 

Father and Mother Group Box:

The Father and Mother group boxes contain the current address information automatically populates based on current address.  The fields can be updated on this window but these changes do not update the system information.

The selected parent is a random selection first looking for a Biological Parent then if none exist or if Parental Rights are terminated then a Adoptive Parent.

Name: This field displays the Parent's first and last name.

Line 1: This field displays the Parent's street address for a physical address or the P.O. Box for a mailing address.

Line 2: This field displays any additional street address information such as an apartment number for a physical address. This is an optional field.

Line 3: This field displays any additional street address information such as an apartment number for a physical address. This is an optional field.

City: This field displays the city where the address is located.

Zip: This field displays the zip code for the address.

Home #: This field displays the Father or Morther's current phone number.

Work #: This field displays the Father or Mother's work phone number.

Are the parent(s) aware of this referral to the Early Development Network:  Select the Yes or No radio button.

Do you sense the parents are willing cooperate with EDN service providers?:  Select the Yes or No radio button.
