Policy Information

WINDOW OVERVIEW:

The Policy Information window is used to enter the specific policy information on the Life Insurance Verification Request form.

FIELD DESCRIPTIONS:

Policy Number(s): Enter the policy number for up to four policies.

Name(s) of Insured: Enter the names of up to four person that may be covered by the policy(s).

ACTION BUTTONS:

	Select the OK button to save the information and return to the Life Insurance Verification Request window.
	Select the Cancel button to close the window without saving any information and return to the Life Insurance Verification Request window.
	Select the Help button to open window specific help.
