Service Need Assessment Notice of Action

WINDOW OVERVIEW:

The Service Need Assessment Notice of Action is a required notice when Assessment Services are denied, terminated, suspended, or reduced.  A copy is sent to the client or client's representative.

How Do I:

Send Service Need Assessment Notice of Action

FIELD DESCRIPTIONS:

Person Group Box:

Person:  This field displays the name of the person who is receiving the services.

Effective Date:  Enter the date (MM-DD-YYYY) the action will be effective.  If the action is not eligible, an effective date is not needed.

Notice Type List Box:

Select one of the notice types listed.

Changed:  Select this radio button to generate a change in services notice.

Denied:  Select this radio button to generate a denial notice.

Reduced:  Select this radio button to generate a reduction in services notice.

Discontinued:  Select this radio button to generate a notice that discontinues services.

Not Eligible:  Select this radio button to generate a not eligible notice.

Program Service List Box:

Select the appropriate
SSAD/CHORE

PASS/PASProgram and Service in the drop down list by selecting the drop down button.

Reason for Action(s) Box:

Enter the reason for the action that is taken.  This will be displayed in the Reason for Action area of the notice.

Manual Reference:  Enter the manual reference that supports the action taken.  It will displays on the notice.  This is an optional field.
