bring the provider name back to the list on this window.The Service Needs Assessment Plan Notification identifies the tasks that a client needs assistance with to perform daily activities and notifies the client and provider of the time allowed for each task. Both the client and each provider identified receive a copy of the notice.
The information on the notice is gathered from the information entered on the Service Needs Assessment Questions and Service Needs Time Assessment windows.

Person Group Box:
Person: This field displays the name of the person who is receiving the services.
Provider List Box:
This box displays the list of providers. To
add a provider to the list select the arrow. The
Search Organization window opens to find or add the
provider organization. When
the provider is located use the blue return arrow to
bring the provider name back to the list on this window.

The Remove button is enabled when a provider line is highlighted in the provider list. Use it to remove the provider from the list when they are no longer providing services and should not receive a notice. |
