. This worker is responsible for or will
complete the Assessment of Placement Safety and Suitability.The Detail SDM Assessment of Placement Safety and Suitability window is used to enter assessment information regarding the suitability of the foster home for all the children placed in the home.
Org Name: This field displays the name of the Organization for whom the assessment will be completed.
Org ID#: This field displays the unique identification number assigned to the selected Organization.
Assessment Group Box:
Date: This field is entered by the worker.
Ref. Date: This field displays the referral date; it is the date the foster home was referred for the assessment. It is normally the date of the intake.
ID: This field displays the assigned random number for the assessment.
Completed By:
This field displays the worker selected by using the Out Arrow. This worker is responsible for or will
complete the Assessment of Placement Safety and Suitability.
Office: This field displays the office of the worker displayed in the Completed By field.
Placement Decision: This field will be blank until after the worker completes the Placement Decision section.
Intake: It is not a requirement to have an intake tied to an Assessment of Placement Safety and Suitability. If an Intake needs to be tied, click on the Out Arrow, which will open the Intake Search window for the worker to find and select the intake to tie to the Assessment of Placement Safety and Suitability.
Status: This field displays the current status of the assessment.
as of: This field displays the date the assessment was placed in the status displayed.
Facility Types: This field displays a list of Foster Home Facility Types, and will include the facility types that are currently active for the organization.

Persons Involved in Assessment Group Box:
Name: This field displays the name(s) of the persons involved in the assessment.
Role: This field displays the role of the persons in the assessment.
Birthdate: This field displays the date of birth for each person in the assessment.
Placement Change: This field displays a Yes(Y) for each foster child involved in the assessment that was removed from this foster home as a result of this assessment.

Select this button to remove a person from the assessment. The Assessment must be in Draft, Revisions Required or Administrative Reopen Status |

Select this button to change the role of persons that are currently in the role of "Caregiver" or "Other".The role of Foster Children cannot be changed. |

Reviewed By Group Box:
Supervisor: This field displays the name of the supervisor who reviewed the Safety Assessment. The field is blank until a review is completed.
On Behalf of: This field displays the name of the supervisor responsible for the review when another supervisor completed the review.
