Caller/Reporter Information

WINDOW OVERVIEW:

The Caller/Reporter Information window is used to document the identity of the caller/reporter of the Intake report.

Intake Overview

FIELD DESCRIPTIONS:

First Name: The first name of the Caller/Reporter.

Middle Name: The middle name of the Caller/Reporter.

Last Name: The last name of the Caller/Reporter.

Ext: The extension for the name of the Caller/Reporter.

Sex: The sex of the Caller/Reporter (Female, Male or Unknown).

Birth Date: The birth date of the Caller/Reporter.

SSN: The Social Security Number of the Caller/Reporter.

Source: Select the appropriate source from the drop down. The source indicates the relationship of the Caller/Reporter to the situation.

Note: To remove the Caller/Reporter from the Detail Intake window, selection Actions>Remove Caller/Reporter. A confirmation window will display. Selecting OK will remove the Caller/Reporter. Select Cancel to return to the Detail Intake window without making any changes.

ACTION BUTTONS:

	Select the OK button to save any Caller/Reporter information entered on the window.
	Select the Cancel button to close the window. No information will be saved.
