Detail Safety Plan Person Involvement

WINDOW OVERVIEW:

The Detail Safety Plan Person Involvement window opens when the Add/Update button is selected on the Detail Safety Plan windows  The window is used to document the role and relationship of all persons in the Safety Plan.

Safety Plan Overview

FIELD DESCRIPTIONS:

CFS Case Name:  This field displays the name of the CFS Program Case.

Mode:  The current window mode displays under the close icon.

Safety Plan Begin Date:  This field displays the date the worker began the safety plan.

Involvement History List Box:

This box displays the persons involved in the Safety Plan.

Name:  This column displays the First and Last name of all persons involved in the safety plan.

Birth Date:  This column displays the person's date of birth.

Resides in Home:  This column displays IN if the person resides in the home or OUT if the person resides somewhere other than the home.

NCP:  This column displays a Y if the person is a non-custodial parent and a N if they are not.

Safety Monitor:  This column displays a Y if the person is a safety monitor and a N if the person is not.

Relative:  This column displays a Y if the person is related to the child and a N if the person is not a relative.

Description:  This column displays any relationship description entered for the person.

ACTION BUTTONS:

When a person line is highlighted in the Involvement History list box the Remove button is enable.  The person is also listed in the Add/Update list box.

 Select the Remove button to take a person out of the involvement list if they have been entered there incorrectly.  The person is deleted from the list if the assessment has not been saved.

Add/Update Involvement List Box:

This box is used to add new person to the list of persons involved in the Safety Plan or to update information for persons already listed in the Involvement History list.

Name:  This column displays the First and Last Name of the person(s) highlighted in the Involvement History list box or any persons added to the Safety Plan..

Birth Date:  This column displays the person's date of birth.

ACTION BUTTONS:

 Select the Search arrow to open the Add Person window.  This is used to add additional persons involved in the Safety Plan.
 The Add button is enabled when a person has been added to this list using the Arrow above.  The person moves from the Add/Update Involvement List Box to the Involvement History List Box.
 The Update button is enabled when a person in the Involvement History List Box is highlighted and the name is then moved to the Add/Update Involvement List Box to be updated.  Select this button to return the updated information to the Involvement History List Box.

Resides in Home:  Check this box if the person resides in the home.

NCP:  This box should be checked it the person is a non-custodial parent.  Only Bio or Adoptive parents should be given this role.

Relative:  Check this box if the person is related to the child in the Safety Plan.

Safety Monitor:  Check this box if the person is the Safety Monitor in the Safety Plan.

Relationship to Family:  Enter any additional descriptive information about the person in the optional field.
