Enter Effective End Date

WINDOW OVERVIEW:

The Enter Effective End Date window is used to document the date the Safety Plan ends.  The window is opened when the Enter end Date option is selected on the Detail Safety Plan Actions drop down menu.

FIELD DESCRIPTIONS:

End Date:  Enter the date (MM-DD-YYYY) the safety Plan ends.

ACTION BUTTONS:

	Select the OK button to save the end date information.
	Select the Cancel button to close the window without saving the end date information.
