Detail SDM Reunification Assessment

WINDOW OVERVIEW:

The Detail Reunification Assessment window opens when the Reunification Assessment button is  selected on the SDM Household Summary Detail window.

FIELD DESCRIPTIONS:

CFS  Case Name:  This field displays the Program Case Name for the CFS program case.

MC #:  This field displays the Master Case number.

Household Name:  This field displays the name of the person identified as the head of the household.

Referral Date:  .This is the Referral Date entered by the worker when the household was created. This is normally the date of the Intake.

Safety Plan Group Box:

Assessment Date:  Enter the date (MM-DD-YYYY).  the worker started the assessment process.  This date must be the current date or earlier, but cannot be prior to any previous assessments.  Nor can this date be prior to the Referral Date. .

ID Nbr:  This field displays the randomly assigned ID for the Assessment.

Completed By:  This is an out select to identify the person responsible for completing the SDM Detail Reunification  Assessment.

Office:  This is the office of the worker selected as the completed by.

Status:  This field displays the Reunification Assessment status.

as of:  This field displays the current assessment status begin date.

Final Risk Level:  This displays the Risk Level determined after the Scoring/Override section for the Assessment has been completed.

Scored Risk Level:  This will display on the screen when the Safety Re-assessment section is completed.

Safety Decision:  This will appear on the screen after the worker has completed enough information on the safety assessment that will apply a Safety Decision. The minimum to have this field display is to either have the Safety Threat selection of "No Safety Threat" selected or have a safety threat and a safety intervention selected.

	Select this button to open the SDM Reunification Assessment-Risk Index window.
	Select this button to open the SDM Parenting Time Summary window.
	Select this button to open the SDM Reunification Assessment -Safety Threats/Decision window.
	Select this button to open the SDM Reunification Assessment -Score Overrides window.
	Select this button to open the Detail SDM Safety Plan.
	Select this button to open the SDM Reunification Assessment-Recommendations window.
	Select this button to open the Reunification Assessment Status History dialog box.

Persons Involved in Assessment Group Box:

Name:  This field displays the names of the persons involved in the SDM Safety Assessment.

Role:  This field displays the same roles that the person has in the SDM household at the time the SDM Safety Assessment is created.

Birthdate:  This field displays the date of birth for each person.

ACTION BUTTON:

Select this button to change the Removed status of the highlighted child in the Persons Involved in the Assessment group box.

FIELD DESCRIPTIONS:

Reviewed By Group Box:

Supervisor:  This field displays the name of the supervisor who reviewed the Safety Assessment.  The field is blank until a review is completed.

On Behalf of:  This field displays the name of the supervisor responsible for the review when another supervisor completed the review.'

ACTION BUTTON:

Select the Review Narrative button to open the SDM Narrative window.
