Detail SDM Safety Plan

WINDOW OVERVIEW:

The Safety Plan button is enabled on the Detail SDM Safety Assessment window if the Safety Decision for the Safety Assessment is either "unsafe" or "conditionally safe".  

This window opens when the Safety Plan button is selected on the SDM Safety Assessment window or the List SDM Safety Plan window.

FIELD DESCRIPTIONS:

CFS Case Name:  This field displays the Program Case name for the CFS Program Case.

Master Case ID:  This field displays the Master Case number.

Household Name:  This field displays the name of the person identified as the head of the household.

Referral Date:  .This field displays the Referral Date entered by the worker when the household was created. This is normally the date of the Intake.

Safety Plan Group Box:

Begin Date:  This displays the begin date of the Safety Plan.

End Date:  This date will appear when a new Safety Plan is created or when a new Safety Assessment is finalized and the Safety Determination is"safe".

ID:  This field displays the unique randomly selected number assigned to the SDM Safety Plan when it is created.

Completed By:  This field displays the User ID of the worker who completed the Safety Plan.  

Office:  This displays the office of the worker in the "Completed By" field.

Safety Decision:  This Safety Decision displays based on the SDM Safety Assessment that is being created.

Status:  This field displays the status of the Safety Plan.

as of :  This field displays the date the Safety Plan was placed in it's current status.

ACTION BUTTONS:

:  Select the Arrow to open the Search Office Position window to User ID  of the person completing the Safety Plan.

Select the Safety Threat Narrative button to open the SDM Safety Plan Narrative window.

Persons Involved in SDM Safety Plan Group Box:

Name:  This displays the names of all persons in the household/safety assessment.

Birth Date:  This displays the date of birth for each person listed.   It will be blank if there is no date of birth for the person.

ACTION BUTTON:

Select this button to assign the role of Safety Monitor to the person highlighted in the Persons Involved in SDM Safety Plan Group box.

Safety Monitors in SDM Safety Plan Group Box:

Person:  This box displays all persons selected to be a monitor in the SDM Safety Plan.

Organization:  This box displays  the Organizations identified to be used for Safety Monitoring.

Case Worker:   This box displays the name of the case worker identified to be a monitor.  

ACTION BUTTONS:

:  Select the Arrow to open the Person Search window or the Search Organization window, or the Search Staff Person window to select either a person, Organization or Case Worker to add as the Safety Monitor.

	Select the Remove button to remove the person, Organization or Case Worker as the SDM Safety Monitor.
	Select the Suitability button  open the SDM Safety Plan Monitoring Assessment window.
	Select the Safety Monitor History button to open the SDM Safety Plan Monitor history window.
	Select the Progress Narrative button to open the SDM Safety Plan Progress Narrative window.
