SDM Household Summary Detail

WINDOW OVERVIEW:

The Structured Decision Making (SDM) Household Summary/Detail window displays the name(s) of all persons involved in the Initial CFS Assessment.

FIELD DESCRIPTIONS:

CFS Case Name:  This field displays the Program Case Name for the CFS Program Case..

Master Case ID:  This field displays the unique, system-assigned number used to identify each Master Case.

Household Name: This field displays the name of the person identified as head of the household..

Referral Date:  This field displays the date the case was referred for the Assessment.

Status:  This field displays the status of the Household.

Status Begin Date:  This field displays the begin date of the status.

Create Date:  This field displays the date the Assessment was created.

Created By:  This field displays the User ID of the person who created the Assessment.

Household ID:  This field displays the unique system assigned ID number for the Household.

ACTION BUTTONS:

	This button is enabled once an assessment is selected.   Select this button to change the status of the assessment selected.
	This button is enabled once an assessment is selected.  Select this button to view the history of the assessment.
	This button is enabled once an assessment is selected.  It opens the SDM Household Overview Summary window.
	Select this button to open the SDM Safety Assessment.
	Select this button to open the SDM Safety Plan.
	Select this button to open the SDM Risk Assessment.
	Select this button to open the SDM Prevention Assessment.
	Select this button to open the SDM Family Strength and Needs Assessment.
	Select this button to open the SDM Reunification Assessment.
	Select this button to open the SDM Risk Re-Assessment.
	Select this button to open the List SDM Case Plan window. You will open the Detail SDM Case Plan window by selecting the appropriate Case Plan shown on the window list.
	Select this button to open the Safety Organized Practice Overview Summary window.
	Select this button to open the Search Narrative window.

SDM Household Group Box:

Name:  This field is blank until the household members and their roles are saved on the SDM Household Detail window.  The name will be the name of the person selected to be the Primary Caregiver or the Household Name.

ID:  This field is populated with the unique Person Number of the Primary Caregiver or Household Name.

Referral Date:  This box is displays the referral date entered on the SDM Household Detail window.

Name:  This field displays the first and last name of the person in the SDM Household.

Role:  This field displays the role of the person in the SDM Household.

Role Begin Date:  This field displays the begin date of the role.

ACTION BUTTONS:

	This button is enabled when a assessment is selected.  Select the button to add household members or non-household members to the assessment on the SDM Household Detail window.
	This button is enabled when an SDM household member is selected.  Select this button to change the person's role in the Household.
	This button is enabled when an SDM household member is selected.  Select this button to remove the person from the SDM household.
	This button is enabled when an SDM household member, other than the Primary Caregiver, is selected.  Select this button to change the person's role to out of household.
	This button is enabled when an SDM household member is selected.  Select this button to open the SDM Household Person Role History dialog box to view historical information about the person's role in the household.
