SDM Intake Screening

WINDOW OVERVIEW:

The SDM Intake Screening tool is used to help determine the Initial Screening Decision for an Intake.  The decision is based on the selections made for the Vulnerable Adult Status and the options selected from the Screening and Overrides questions.

FIELD DESCRIPTIONS:

Intake Name:  The name of the person listed on the Intake.

Intake Type:  The type of Intake.

Intake Number:  A system generated number assigned to the Intake.

Received Date:  The date the Intake was received.

Screening Detail Box:

Initial Screening Decision:  The initial decision that the system determines based on the answers to the questions within the Intake Screening tool.

Final Screening Decision:  The final decision that the system determines based on the answers tot he questions within the Intake Screening tool.

Cross Report Required:  This field will indicate whether a cross report should be made to CPS/APS.

Vulnerable Adult Status:  Answer Yes or No to each question listed.  

ACTION BUTTONS:

 Select this button to open the SDM Screening Detail window.
Select this button to open the SDM Intake Screening Overrides window.
 Select this button to open the Intake Notification pop up window.
Select this button to open the SDM Response Priority window.
 This button is enabled when an SDM household member is selected.  Select this button to open the SDM Household Person Role History dialog box to view historical information about the person's role in the household.

When this button is active, selecting the button will take you to the Intake Rescreen Detail window.

Note: This button becomes active when the Intake needs to be closed and in a Status that requires Assessment (Accept for Initial Assessment, Accept for Out of Home Assessment, APS Investigation, ORG Related Investigation, Accept for Self-Neglect Assessment, Unable to Identify-Accepted, Alternative Response, Accept for Placement Assessment, Accept for DHHS Facility Investigation)
