Search By SSN

The Search by SSN filter on the Records tab is used to search for Vital Records information using the person's social security number. The field defaults to the selected person's SSN from NFOCUS and cannot be changed. If you are searching Birth Certificate, the selected person's Birth Date is included on the search and cannot be changed.

FIELD DESCRIPTIONS:

Search Records Box:

Search by:Select the SSN radio button to search records by SSN.

SSN:This field displays the SSN of the person selected in NFOCUS. It is used in the search for certificates and cannot be updated.

Certificate Type:Select the certificate type you want to search for (Birth, Death, Divorce, or Marriage).

Birth Date:This field displays the birth date of the NFOCUS user used for the Birth Certificate search only. This field cannot be updated.

Results List Box:

When the information does not match Vital Records, the message reads:

When there is a match, the List Box displays:

Certificate Number:This column displays the certificate number. Click on the certificate number to view the details of the certificate.

Certificate Type:This column displays the type of certificate(s) found.

Birth Certificate Information:

Child:This column displays the last and first name of the child as listed on the birth certificate.

Mother:This column displays the last and first name of the child's mother as listed on the child's birth certificate.

Date:This column displays the child's date of birth.

Death Certificate Information:

Deceased:This column displays the last and first name of the person as listed on the death certificate.

Spouse:This column displays the last and first name of the person's spouse.

Date:This column displays the date of death.

Divorce Certificate Information:

Husband:This column displays the last and first name of the husband as listed on the divorce certificate.

Wife:This column displays the last and first name of the wife as listed on the divorce certificate.

Date:This column displays the date of the divorce.

Marriage Certificate Information:

Groom:This column displays the last and first name of the groom as listed on the marriage certificate.

Bride:This column displays the last and first name of the bride as listed on the marriage certificate.

Date:This column displays the date of the marriage.

ACTION BUTTONS:

	Select the Go button to begin the record search process.
	Select the Reset button to clear all search criteria entered.
