Health Check List

WINDOW OVERVIEW:

The Health Check window opens when the Health Check option is selected from the Goto drop down menu on the Person Detail window.  It is used to view a list of Health Check actions.  If the selected person is not eligible for Health Check a message opens.

The information is taken from the Health Check task in the Expert System.

FIELD DESCRIPTIONS:

Case Person Group Box:

This box displays the eligible person's first and last name.

Health Check List Box:

Date Offered:  This column displays the first date that Health Check was explained and offered.

Date Approved:  This column displays the date the medical eligibility was determined.

Medical Accepted?  This column displays Y
Yes. Health Check referral accepted. or N
No - not accepted. to indicate that the client did or did not accept Health Check services for a medical exam and treatment.

Dental Accepted?:  This column displays Y
Yes. Health Check referral accepted. or N
No - not accepted. to indicate that the client did or did not accept Health Check services for a dental exam and treatment.

Transportation Needed?  This column displays Y
Yes. Health Check referral accepted. or N
No - not accepted. to indicate that the client did or did not need transportation assistance to access either medical or dental exams and treatments:

Appt Needed?:  This column displays Y
Yes. Health Check referral accepted. or N
No - not accepted. to indicate that the client did or did not need assistance arranging an appointment for either medical or dental exams and treatments.
