7-008.02 Service Coordination Activities
The worker must gather, compile, and document information necessary to develop a service plan within 15 days of case status determination.
The following guideline is offered for determining level of involvement: no need, low, moderate, and high. This information may be useful for service planning, but this level of detail is not required for determining whether criteria are met in this domain. A person with NO NEED • Memory: Intact memory. Does not need cueing. • Orientation: Fully oriented to person, place, and time. • Communication: Able to communicate information in an intelligible manner. Able to understand information conveyed. • Judgment: Solves everyday problems well. Makes appropriate decisions. A person with LOW need • Memory: Slight, inconsistent deficits. May need occasional cueing. • Orientation: Only slight problems with time relationships. • Communication: Minor/inconsistent problems with name/word finding, or with expressive or receptive understanding. A person with MODERATE need: • Memory: Has moderate memory loss, especially for recent events. Needs frequent cueing. • Orientation: Occasional disorientation to person/place/time, but able to function semi-independently in familiar surroundings. • Communication: Moderate or consistent problems with name/word finding and with expressive or receptive understanding. • Judgment: Moderate difficulty in solving problems. Judgment occasionally to frequently poor. Needs monitoring or guidance in decision making. A person with HIGH need: • Judgment: Mild impairments in solving problems. Occasional poor judgement, especially in complex or unfamiliar situations. • Memory: Has severe memory loss, retaining only highly learned information. Needs continual reminding by others. • Orientation: Frequent disorientation to person/place/time, even in previously familiar surroundings. Oriented only to person. Totally disoriented. • Communication: Severe or consistent problems with communication and understanding. Unable to communicate intelligibly. • Judgment: Severe impairment in solving problems. Consistently makes poor decisions. Unable to solve simple problems or make appropriate judgements. |
