Birth Certificate Request
WINDOW OVERVIEW:
The Birth Certificate Request window is accessed under the New Request tab within Vital Records. You need to fill out the information on the window and click submit to request a birth certificate. After the request is submitted, you can track the request within the Requests tab.

FIELD DESCRIPTIONS:
Request Type:Click on the appropriate radio button.

Child Box:
First Name:Enter the child's first name. This is a required field.
Middle Name:Enter the child's middle name if they have one.
Last Name:Enter the child's last name. This is a required field.
Ext: Enter the child's name extension if they have one.
Birth
Date:Enter
the child's date of birth (MM-DD-YYYY). This
is a required field. Select
the calendar icon
to open a calendar
of the current month.
City:Enter the name of the city where the child was born. This is a required field.
State: If Out of State is selected enter the name of the state where the child was born. For Nebraska requests this field is completed.
County: Enter the county where the child was born.
Foreign Country: Enter the foreign country where the child was born if out of state and applicable.
Father Box:
First Name:Enter the child's fathers first name. Required for out of state requests.
Middle Name:Enter the child's father's middle name if he has one.
Last Name:Enter the child's fathers last name. Required for out of state requests.
Ext:Enter the child's father's name extension if he has one.
Mother Box:
First Name:Enter the child's mother's first name. Required for out of state requests.
Middle Name:Enter the child's mother's middle name if she has one.
Last Name:Enter the child's mother's last name. Required for out of state requests.
Ext:Enter the child's mother's name extension if she has one.
Maiden Name:Enter the child's mother's maiden name.
