EBT Card Request

WINDOW OVERVIEW:

This window is used to request that an EBT Replacement Card be sent to the SNAP program case person.  All rows must have a Response indicator of Yes or No. Replacement EBT Cards may be issued to clients during the interview.

FIELD DESCRIPTIONS:

Case Name:  The name of the program case person.

Program Case ID:  The program case ID number.

EBT Card Last Request Date:  The last date an EBT card was requested for this case person.

Client Response:  This column indicates the answer selected in the Response Box. (Yes or No)

Answer Yes or No to the question 'Do you have a Nebraska Electronic Benefits (EBT) card for the Supplemental Nutrition Assistance Program (SNAP)?.
