Family Relationships Verifications

WINDOW OVERVIEW:

The Family Relationships Verifications window opens when the Update Verifications option is selected on the actions drop down menu of the Family Relationship List window in the Mainframe. It is used to document how the relationships between the Agency Related Persons was verified.

FIELD DESCRIPTIONS:

Verification Group Box:

Source: Select the verification source from those listed in the drop down list.

Date: The date will be completed with the current date but may be changed to a previous date.

Relationship List Box:

Agency Related Person row ( the headings in the top row correspond to the ARP information for the person with the name in CAPITAL letters with the - sign before the name)

Name: This column displays the Last the First name of the Agency Related Person.

Age: This column displays the age of the Agency Related Person.

Birthdate or EDD: This column displays the date of birth (or expected date of delivery for an unborn) for the Agency Related Person.

Number: This column displays the person's unique ARP number.

Relationship row (the headings for the lower row(s) correspond to information about the person(s) who are related to the ARP).

Relationship type: This column displays the marital status of the ARP or the type of relationship the ARP has with the person in this row.

Name: This column displays the name of the person(s) related to the ARP.

Birthdate or EDD: This column displays the date of birth (or expected date of delivery for an unborn) for the person related to the ARP.

Begin Date: This column displays the date the relationship started.

Verification Source: This column displays the document used to verify the relationship.

Verification Date: This column displays the date the relationship or marital status verification was received.

Number: This column displays the persons ARP number
