The Earned Income Verification Request window is used to create a verification request form for a source of earned income. The information entered will print out on the request form that is then printed locally and should be mailed with a signed release of information form and self addressed stamped envelope.

Non System Provider: Select this option if the provider is not found with the Search Organization window. The Non-System Information window opens.
Sent to Provider: Select the
search arrow
to open the Search Organization
window. This is where the provider can be selected and returned to this
field.
Client Name Group Box:
Highlight the employed person on the list.
Household: This column displays the first and last name of the household person.
SSN: This column displays the SSN of the household member.
Status: This column displays the status of the household member.
Wage Information Requested for Period of Group box:
Begin Date: Enter the date the verification request should begin.
through: Enter the date the verification request should end.
