Notice Template ADC

WINDOW OVERVIEW:

The Notice Template ADC is used to create notices for Aid to Dependent Children Program Cases.

FIELD DESCRIPTIONS:

ADC Name: This box displays the name of the person selected in the Master Case Person box on the Notice Template window.

Effective Date: Enter the date (MM-DD-YYYY) the notice information will be effective.

Radio Buttons:

Selecting a radio button for the appropriate notice action enables the required fields.

Begin: Select this radio button and enter the amount of the grant payment. If the grant will change for a subsequent month enter the effective date of the change and the new grant payment amount.

Increase: Select this radio button and enter the amount of the increased grant payment.

Decrease: Select this radio button and enter the amount of the decreased grant payment.

Stop: Select this radio button when the ADC benefits will stop.

Pending: Select this radio button when the ADC case is pending.

Not Eligible: Select this radio button when the ADC case is not eligible.

No Change: Select this radio button when there is no change to the ADC eligibility or grant payment.

ACTION BUTTONS:

	Select the OK button to complete the ADC Notice Template.
	Select the Cancel button to close the window without creating a ADC Notice Template.
	Select the Help button to open window help.
