LAST NAME: This field displays the last name of any master case person with established Medicare information.
FIRST NAME: This column displays the first name of the master case person with established Medicare information.
DOB/EDD: This column displays the person’s date of birth.
AGE: This column displays the person’s current age.
NUMBER: This column displays the person's Agency Related Person number.
Medicare Type: This column displays the Medicare eligibility according to type:
Claim Number: This column displays the Medicare beneficiary’s claim number with alpha suffix (alpha suffix is required).
Begin Date: This column displays the begin date for this instance of Medicare coverage.
End Date: This column displays the end date for the instance of Medicare coverage.

The Update button is enabled when Medicare data has been entered onto the system. Select ‘Update’ to make needed changes to information entered previously by the user to the system. Updates cannot be made to Interface created information. |

The Delete button is enabled when Medicare information has been previously entered. Select “Delete’ to remove incorrect user placed medical impairment information. |
