The List Provider Matching window displays the list of Organizations that meet the criteria selected on the Search Provider Matching window.
To print the list select the print option in the menu bar. The Print window opens.
Search Fields Group Box:
Facility Type: This field displays the facility type selected on the Search Provider Matching window.
Status: This field displays the facility status selected on the Search Provider Matching window.
Organization List Box:
This box lists the organizations that meet the criteria selected on the Search Provider Matching window. Scroll to the right to view all of the information about the Organization.
Organization: This column displays the name of the Provider Organization.
County: This column displays the county where the Provider Organization is located.
Slots: This column displays the number of slots available at the Provider Organization.
School District: This column displays the school district where the Provider Organization is located.
Family's Ethnic Background: This column displays the ethnic background of the Provider Organization family.
Cross Cultural: This column displays the Y or N cross cultural indicator for the Provider Organization family, indicating whether or not the provider is willing to have a child of another culture placed with them.
Language: This column displays a second language spoken by the Provider Organization family.
Family's Religion: This column displays the religious affiliation of the Provider Organization family.
Physical Address/Telephone Number Group Box:
When an Organization line is highlighted in the list the Physical Address of the Provider Organization displays in this group box:
Home: This field displays the home telephone number of the Provider Organization.
Work: This field displays the work telephone number of the Provider Organization
