Search Provider Matching

WINDOW OVERVIEW:

The Search Provider Matching window is used to select the criteria to be used in a search for an appropriate placement for a Child Welfare case.

The search can be processed using the criteria in the Organization group box, the Location group box, and or the Child group box.

The Facility Type and Status group box must be completed for all searches.

Provider Matching Overview

FIELD DESCRIPTIONS:

Organization Group Box:

Name: Enter the name of the Organization.

OR

Org ID: Enter the Organization's N-FOCUS identification number.

How Do I:

Match by Organization

Facility Type and Status Group Box:

Facility Type: Select the appropriate type of facility from those in the drop down list

Status: Select the
Inquiry
In Progress
Active
Hold
Closedstatus for the facility from those in the drop down list.

Family Language: Select the preferred language that the family speaks.

# of Slots Available: Enter the number of slots that are available for the Facility Type.

Licensing Agent: Enter the name of the Licensing Agent.

Location Group Box:

Select one or more of these fields

County(s): Highlight one to three counties from those in the drop down list.

City: Enter the name of the city.

School District: The school district list enables when the name of a city is completed. The list of the school or school districts is then specific to the city.

Zip Code: Enter a zip code for the search.

How Do I:

Match by Location

Child Group Box:

A location or organization name must be completed along with the information in this group box.

Sex: Select the Male or Female.

Age: Enter the number of the age and select the
Years
Months
Weekstime frame from the drop down list

Conditions: Highlight up to five characteristics of the child.

How Do I:

Match by Child Information

ACTION BUTTONS:

	Select the Search button to begin the search for a provider that meets the selected criteria. If there is more than one organization that meets the criteria the List Provider Matching window opens.
	Select the New button to open the Detail Organization window where a new organization can be added.
	Select the Clear button to remove any selected criteria.
	Select the Cancel button to close the window and return to the N-FOCUS Main Menu.
	Select the Help button to open the help information for the current window.
