icon.The Placement Agreement combines the Placement Agreement and Child Disclosure forms. The Placement Agreement window can be accessed from the Detail Placement window by selecting the Placement Agreement icon.

Child Group Box:
This box displays the name of the child on the Placement window.

Parents/Caretakers List Box:
Name: This column displays the first name, middle initial, last name, and name extension of the child's parent(s) and or caretaker(s)
Relationship: This column displays the parent or caretaker relationship to the child.
Parental Rights: This column displays a Y(es) or (N)o indicator to show whether parental rights are intact.
Placement Effective Date: The date the placement became effective.
Placement Organization: The name of the placement organization.

Select the Search arrow to open Search Organization window. This is used to select the name of the placement organization. |
Case Workers List Box:
Case Manager: This field displays the first name, middle initial, and last name of the case manager assigned to the case.

Select the Search arrow to open Search Office Position window. This is used to select a new case manager for the disclosure form. |
Supervisor: This field displays the first name, middle initial, and last name of the assigned case manager's supervisor.

Select the Search arrow to open Search Office Position window. This is used to select a another supervisor for the disclosure form. |
After Hours, Weekend and Holiday Phone: Enter the emergency telephone number for the case manager.
