The Detail Safety Plan Participant Assessment window is used to document the suitability of the person selected to be the Safety Monitor. It opens when the Safety Monitor person line is highlighted on the Detail Safety Plan and the Suitability button is selected.

Safety Monitor: This field displays the name of the person selected as Safety Monitor on the Detail Safety Plan window.

Select the Yes button to indicated that the Safety Monitor meets the highlighted criteria. |

|
Select the Nos button to indicated that the Safety Monitor does not meet the highlighted criteria. |
Use the Enter Key on the keyboard to repeat a selected answer for each criterion.
Participant Assessment Criteria List Box:
This list displays each of the criteria to be answered. Remember to scroll down to see all the questions in the list
Exception Group Box:
Request Date: Enter the date (MM-DD-YYYY) and exception was requested.
Exception Approved Group Box:
Yes: Select the Yes radio button if the exception was approved.
No: Select the No radio button if the exception was not approved.
Review Date: Enter the date the exception was reviewed.

Select the Search Arrow to open the Search Office Position window to find the name of the staff person that reviewed the exception request. |
