Detail Trust Fund Request

WINDOW OVERVIEW:

The Detail Trust Fund Request window opens when there is only one request that meets the search criteria on the Search Trust Fund Request window. The window is used to view the details of the selected request, or to create a new request.

Mandatory fields for a new request have underlined field titles.

FIELD DESCRIPTIONS:

Mode: The current Mode is displayed under the Close icon.

Person Group Box:

Name: This field displays first and last name of the state ward. The field will be blank when creating a new account. This is a mandatory field.

SSN: This field displays the social security number of the state ward. The field will be blank when creating a new account.

Birthdate: This field displays the state ward's date of birth. The field will be blank when creating a new account.

Search To add a state ward to a request select the search arrow to open the Person Search window. When the selected person is returned to this window the Name, SSN, and Birthdate fields will populate.

Request Date: This field displays the date the request was made. This field displays after the request is saved.

Status: The status and status date display the current status of the request and the date that status became effective.

Request Group Box:

Either the Desc and Amount or the Monthly Maintenance and Begin Date are mandatory fields

Desc: Select the description of the request from those in the drop down list.

Amount: Enter the dollar amount of the request.

Monthly Maintenance: When the request is to begin Monthly Maintenance (yes) or end Monthly Maintenance (no) select the appropriate radio button and begin date.

Begin Date: Enter the Begin Date (MM-DD-YYYY) when monthly maintenance begins or ends.

Requested By: This field displays the ID of the person making the request. This field displays after the request is saved.

Reviewed By: This field displays the ID of the Financial Responsibility staff person who processes the request. This field displays after the request is processed.

Payee Group Box:

Type: Select the type of payee from those in the drop down list.

Number: Use the Search arrow to open the Search Organization or Person Search window to find the name of the organization or person. When selected, the system will return the organization related or person related name and identification number.

Mail To: Select where the payment should be mailed.

Name: This field displays the name of the Organization or Person related to the payee number above.

Case Person The Case Person icon is enabled when a person is selected as the payee. Select this icon to open the Person Detail window for the selected person.
Organization The Organization icon is enabled when an organization is selected as the payee. Select this icon to open the Detail Organization for the selected organization.

Comments: Either the requester or the reviewer may enter comments about the request.

Ward Account List Box:

This box identifies all accounts for the State Ward.

Acct: This column displays the number of the account .

Type: This column displays the type of account.

Status: This column displays the status of the account.

Name: This column displays the Bank name of the account.

Pers Avail: This column displays the personal assets available balance.

Available: This column displays the account available balance.

Balance: This column displays the current account balance.
