The Search Trust Fund Request window opens when the Request icon is selected from the State
Ward Sub Menu.
The Search Trust Fund Request window is used to collect the search criteria used to display detailed information about a Trust Fund Request or to begin the process to create a new request.
If only one request meets the search criteria the details will display on the Detail Trust Fund Request window.
If more that one requests meets the search criteria the list of requests will display on the List Trust Fund Request window.
Request Group Box:
Number: Enter the request number if known to go directly to the Detail Trust Fund Request window. All other fields are disabled when this search criteria is entered.
OR
Requested By Group Box:
Use one or any combination of Staff Person, Reviewed By, or Ward to search for a request when the request number is not known.
Staff Person: Use the arrow to open the Search
Staff Person window where the process to locate the name of the staff
person who initiated the request begins.
AND/OR
Reviewed By Group Box:
Staff Person: Use the arrow to open the Search
Staff Person window where the process to locate the name of the staff
person who reviewed and processed the request.
Ward Group Box:
Name: Use the arrow
to open the Person Search
window where the process to locate the name of the ward begins.
SSN: The Ward's Social Security Number will display when the Ward has been selected.
Account: Enter the State Ward Account number if known.
AND
Request Group Box:
Status: Select the appropriate request status from those in the drop down list.
Exclude Paid Requests: This indicator is automatically selected when searching by the Status of "All". It may be deselect, if desired. It is used to narrow the search to avoid system performance issues due to the size of the list.
Exclude Canceled Requests: This indicator is automatically selected when searching by the Status of "All". It may be deselect, if desired. It is used to narrow the search to avoid system performance issues due to the size of the list.
Request Date Group Box:
These dates are use to narrow search and improve system performance due to the size of the list.
Begin: Enter the first date (MM-DD-YYYY) for the search. This date must be the current date of earlier.
End: Enter the last date (MM-DD-YYYY) for the search. This date must be after the Begin date.

Select the Search button to begin the search for the request based on the selected criteria. If only one request is found that meets the search criteria the Detail Trust Fund Request window opens. If more than one request is found that meets the search criteria the List Trust Fund Request window opens. |

Select the New button to open a blank Detail Trust Fund Request window where a new request can be added. |

Select the Clear button to remove any search criteria and remain on the search window. |

Select the Cancel button to close the window and return to the State Ward Sub Menu. |
